HARTFORD, Conn. (AP) - A new report from a group dedicated to worker safety says several factors, including mayday procedures, contributed to the October 2014 death of a Hartford firefighter.

Kevin Bell became separated from his lieutenant while battling a house blaze and his tank ran out of oxygen. The lieutenant sent out a mayday call that went unacknowledged.

WVIT-TV reports (https://bit.ly/2lCd6UG ) the National Institute for Occupational Safety and Health says contributing factors include fireground tactics, crew integrity and personnel accountability. Air management and personal protective equipment were also cited.



The group says risk assessments should be completed before initial operations and throughout the incident.

The Hartford Fire Department was also told to use a personnel accountability system and make sure firefighters are properly trained in mayday procedures.

The department couldn’t immediately be reached.
